RESUMO

OBJECTIVE@#To study the prognostic value of LPCAT1 in acute myeloid leukemia (AML).@*METHODS@#TaqMan-based reverse transcription-quantitative polymerase chain reaction (RT-qPCR) was used to detect relative expression of LPCAT1 in 214 newly diagnosed adult AML patients and 24 normal controls. Survival functions were estimated using the Kaplan-Meier method and were compared by the Log-rank test. A Cox proportional hazard regression model was used to identify prognostic factors.@*RESULTS@#The expression level of LPCAT1 in adult AML was 34.37%(1.83%-392.63%), which was significantly lower than 92.81%(2.60%-325.84%) of normal controls (P<0.001). The prognostic significance of LPCAT1 was evaluated in 171 non-acute promyelocytic leukemia patients with complete clinical information and prognostic data. Survival analysis showed that the expression level of LPCAT1 had no significant effect on the prognosis of the whole cohort. However, in AML patients with FAB subtype M2 (AML-M2), the 2-year relapse-free survival (RFS) rate of patients with low LPCAT1 expression was 35.4%(95%CI: 0.107-0.601), which was significantly lower than 79.2%(95%CI: 0.627-0.957) of patients with high LPCAT1 expression (P=0.012). Multivariate analysis showed that low expression of LPCAT1 was an independent risk factor for RFS of AML-M2 patients (HR=0.355, 95%CI: 0.126-0.966, P=0.049).@*CONCLUSION@#In adult AML patients LPCAT1 shows low expression. Low LPCAT1 expression is an independent risk factor for RFS in M2-AML patients.

RESUMO

The data mining technology was used to explore the acupoint selection rules for reflux esophagitis (RE), so as to provide references of clinical acupuncture for RE. The clinical literature of acupuncture for RE published before June 2019 was searched in Chinese journal full-text database (CNKI), SinoMed, Wanfang and VIP databases. The literature was selected according to the inclusion and exclusion criteria and acupoint prescriptions were extracted. The software of IBM SPSS Statistics 23.0 and Clementine 12.0 were used for descriptive analysis and association analysis. A total of 46 articles were selected and 60 acupoint prescriptions were extracted. The descriptive analysis indicated that the top five acupoints used for RE were Zhongwan (CV 12), Zusanli (ST 36), Weishu (BL 21), Neiguan (PC 6) and Gongsun (SP 4). The conception vessel, bladder meridian and stomach meridian were the most commonly selected meridians. In terms of specific acupoints, the crossing points, the front- points and five- points were mainly selected, and the acupoints were mainly distributed in limbs and chest-abdomen. The core acupoint combination for RE was "Zhongwan (CV 12) and Zusanli (ST 36)" and the core prescription was "Zhongwan (CV 12), Zusanli (ST 36), Weishu (BL 21) and Neiguan (PC 6)".

Objective To explore the effect and probable mechanism of rosuvastatin preconditioning on inflammatory cytokines interleukin (IL) -1β, IL-6 and tumor necrosis factor a (TNF-α) release in vascular smooth muscle cells (VSMCs) of middle cerebral artery after ischemia-reperfusion. Methods Thirty-six healthy SD rats were randomly assigned into three groups; sham operation group, focal cerebral ischemia-reperfusion group and rosuvastatin preconditioning group. There were 12 rats in each group. At the 24th hour of reperfusion after middle cerebral artery occlusion (MCAO) for 2 hours, the mRNA and protein expression of IL-1β, IL-6 and TNF-α release in VSMCs of middle cerebral artery were detected by Real-time PCR and Western blotting, respectively. Also, the mRNA and protein expression of nuclear factor κB (NF-κB) were measured by Real-time PCR and Western blotting. Results At the 24th hour of reperfusion after MCAO for 2 hours, the mRNA and protein expression of IL-Iβ, IL-6 and TNF-ot were markedly up-regulated in rats of model group; rosuvastatin preconditioning can significantly inhibited overexpression of IL-1β, IL-6 and TNF-ot at the mRNA and protein levels. And the decreasing of mRNA and protein expression of NF-κB was also found in this study. Conclusion Rosuvastatin preconditioning can decrease the release of inflammatory cytokines IL-1β, IL-6 and TNF-α in VSMCs of MCA. It can relieve the inflammatory injury after ischemia-reperfusion in brain. The effect of rovastatin on IL-1β,IL-6 and TNF-ot may be related to the reduction of the expression of NF-κB in VSMCs.

OBJECTIVE@#To investigate the methods for verifying and comparing performance of HIV testing methods and procedure analysis in blood station laboratories so as to meet the requirements of ISO15189 accreditation.@*METHODS@#The performance of HIV test was verified by the automatic ELISA analyzer, the intra- and inter-assay precision was analyzed and evaluated by intra- and inter-assay repeat tests, the compliance rate was verified by the test results of the standard serum plate and the external quality assessment from the Ministry of Health in the past 2 years, the limit of detection was verified through continuous dilution of a known amount of reference serum for internal quality control, the status of the instrument was evaluated by testing one HIV-negative specimen, one HIV-negative with other positive markers, one strong HIV-positive specimen and two weak HIV-positive specimens.@*RESULTS@#The intra- and inter-assay precisions were 5.12% and 16.81% respectively, the compliance rate of the serum plate test was 100%, the compliance of the external quality assessment results was 100%, the limits of detection for HIV was 1.42 NCU/ml, and the consistency of the detection systems was 100%.@*CONCLUSION@#The analytical performance of the HIV test methods and procedures accords with the requirements of the reagent instructions, the comparison of the test systems meets the verification requirements.

OBJECTIVE: To investigate the job burnout status and its influencing factors in personnel of community public health service in Wanzhou District of Chongqing. METHODS: By stratified cluster sampling method,220 workers in community public health service in Wanzhou District of Chongqing City were selected and investigated by Maslach Burn-out Inventory-general Survey. Their job burnout situation and influencing factors were analyzed. RESULTS: The median( the 25 th and 75 th percentiles) of dimension scores of emotional exhaustion,cynicism,and low sense of personal achievement of these 220 workers were 1. 1( 0. 6,2. 4),0. 5( 0. 0,1. 5),4. 7( 2. 3,6. 0) respectively. In each dimension,the ratios of moderate and severe job burnout were 33. 2%,20. 0%,and 40. 9%. Workers whose monthly income was less than2 000 yuan had a higher emotional exhaustion score than those who owed more than 2 000 yuan and more than 2 500 yuan per month( P < 0. 05); workers who work less than 15 years had a higher score in cynicism than those work over 15 years and 25 years( P < 0. 05). The multiple regression analysis results showed that low sense of personal achievement was related to genders( P < 0. 01),and the score of male was lower than that of female. CONCLUSION: Job burnout in workers of community public health service is severe,the degree of job burnout is mainly influenced by monthly income,gender,and length of service.

<p><b>BACKGROUND</b>The treatment of high-grade developmental spondylolisthesis (HGDS) is still challenging and controversial. In this study, we investigated the efficacy of the posterior reduction and monosegmental fusion assisted by intraoperative three-dimensional (3D) navigation system in managing the HGDS.</p><p><b>METHODS</b>Thirteen consecutive HGDS patients were treated with posterior decompression, reduction and monosegmental fusion of L5/S1, assisted by intraoperative 3D navigation system. The clinical and radiographic outcomes were evaluated, with a minimum follow-up of 2 years. The differences between the pre- and post-operative measures were statistically analyzed using a two-tailed, paired t-test.</p><p><b>RESULTS</b>At most recent follow-up, 12 patients were pain-free. Only 1 patient had moderate pain. There were no permanent neurological complications or pseudarthrosis. The magnetic resonance imaging showed that there was no obvious disc degeneration in the adjacent segment. All radiographic parameters were improved. Mean slippage improved from 63.2% before surgery to 12.2% after surgery and 11.0% at latest follow-up. Lumbar lordosis changed from preoperative 34.9 ± 13.3° to postoperative 50.4 ± 9.9°, and 49.3 ± 7.8° at last follow-up. L5 incidence improved from 71.0 ± 11.3° to 54.0 ± 11.9° and did not change significantly at the last follow-up 53.1 ± 15.4°. While pelvic incidence remained unchanged, sacral slip significantly decreased from preoperative 32.7 ± 12.5° to postoperative 42.6 ± 9.8°and remained constant to the last follow-up 44.4 ± 6.9°. Pelvic tilt significantly decreased from 38.4 ± 12.5° to 30.9 ± 8.1° and remained unchanged at the last follow-up 28.1 ± 11.2°.</p><p><b>CONCLUSIONS</b>Posterior reduction and monosegmental fusion of L5/S1 assisted by intraoperative 3D navigation are an effective technique for managing high-grade dysplastic spondylolisthesis. A complete reduction of local deformity and excellent correction of overall sagittal balance can be achieved.</p>

<p><b>OBJECTIVE</b>To observe the therapeutic effects of beta-elemene combined DC/Dribble vaccine in treating mice with hepatic cancer, thus exploring their anti-tumor mechanisms.</p><p><b>METHODS</b>Dentritic cells were derived from Balb/c mice's spleen and their phenotypes were identified. Using hepatic cancer cell line BNL1MEA.7R.1 (abbreviated as BNL) originated from Balb/c mice as target cell, DC/Dribble vaccine was prepared via raising the antigen representing carrier autophagosomes (DRips in Blebs, DRibbles), which were rich in tumor antigen information. The mice previously immunized were divided into 4 groups, i.e., the control group, the beta-elemene group, the vaccine group, and the combined group. The PBS was subcutaneously and intraperitoneally injected to mice in the control group. The beta-elemene was intraperitoneally injected at the daily dose of 50 mg/kg to mice in the beta-elemene group and the combined group for 7 successive days. DC/Dribble vaccine was injected into the lymph node of mice in the vaccine group and the combined group on the 1st day, and DC/Dribble vaccine was subcutaneously injected on the 3rd day and the 5th day. All the mice were sacrificed on the 10th day. Their spleens were obtained sterilely, and the suspension was incubated with or without Dribble. The cells were inoculated for 72 h. The contents of IFN-gamma in the supernatant were measured by ELISA. In addition, the spleen cells obtained from the combined group were incubated with different stimulations for 72 h, which were then divided into the control group, the DRibble group, the DC group, and the DC/Dribble vaccine group. The supernatant of cultured cells were collected and the contents of IFN-gamma were measured by ELISA. The liver tumor-bearing mouse model was established, and then the BNL bearing mice were randomly divided into 4 groups, i.e., the control group, the beta-elemene group, the vaccine group, and the combined group. The treatment ways were the same as the immune ways. The tumor size and the survival period were observed in each group. On the 23rd day the mice were sacrificed. The tumor tissue was stripped and stained by HE staining. The pathomorphological manifestations of the tumor tissue were observed by light microscope.</p><p><b>RESULTS</b>In vitro detection of mice immunized previously by different ways showed that the secretion of IFN-gamma was significantly higher in the combined group than in the rest groups (P < 0.01). The secretion of IFN-gamma was significantly higher in the beta-elemene group and the vaccine group than in the control group (P < 0.01). The spleen cells could be stimulated to secrete a large amount of IFN-gamma in the vaccine group and the Dribble group (P < 0.01). When the beta-elemene was 10 microg/mL as the stimulating dose, the secretion of IFN-gamma obviously increased (P < 0.01). In vivo observation showed that the growth velocity of tumors in mice of the combined group was slowed down. There was statistical difference in the tumor area or the survival period of mice in the combined group, when compared with the other groups (P < 0.01). In HE staining, the surrounding connective tissues of the tumor were wrapped tightly and compactedly, with infiltration of a large amount of inflammatory cells.</p><p><b>CONCLUSIONS</b>beta-elemene combined DC/Dribble vaccine could induce specific immune cells to secrete secretory cells, thus exerting its anti-tumor effect. Its immunological effects might be associated with enhancing the DC antigen presenting function.</p>

Objective To study the rule of aerosol deposition in human upper respiratory tract and analyze the impact of respiratory pattern on aerosol deposition. Methods A computer model of human upper respiratory tract was established first. CFD (computational fluid dynamics) method was then used to numerically simulate the aerosol deposition within the human upper respiratory tract and the rule of aerosol deposition was analyzed. Results The efficiency of aerosol deposition in human upper respiratory tract was improved with the increase of inertial parameter. The breathing intensity and aerosol property had little impact on the pattern of aerosol deposition, which was at most in larynx due to the inertial impact and turbulent dispersion. Under the mode of cyclic inhalation, the aerosol deposition efficiency was higher at unsteady respiratory than that at steady respiratory, at cyclic inhalation than at cyclic exhalation. Conclusions Inertial impact is the main key deposition mechanism for micro aerosol, while turbulent dispersion, secondary flow and recirculation flow have an equally important impact on aerosol deposition in human respiratory tract.

<p><b>OBJECTIVE</b>To compare the clinical results between computer assisted minimally invasive spine surgery (CAMISS) and traditional open fixation surgery which used in thoracolumbar fractures.</p><p><b>METHODS</b>A prospective randomized controlled trial of patients who had undergone surgery for thoracolumbar fracture from January 2006 to March 2011 was performed. The patients were randomly divided into CAMISS group and traditional open treatment group (control group) by random number table. Clinical results were assessed by comparing the following parameters between patients who had undergone CAMISS or traditional open surgery: operative time, estimated blood loss, visual analogue scale (VAS) of the low back pain, the accuracy of pedicle screw, the status and the correction of kyphosis.</p><p><b>RESULTS</b>Forty-seven patients underwent CAMISS, and fifty patients underwent traditional open surgery. The follow-up periods were 3 - 50 months (mean 12 months). According to the preoperative data, the two groups did not differ with respect to age, gender, marriage, occupation, mechanism of injury, classification of fracture, preoperative VAS scores of the low back pain, preoperative functional spine unit (FSU) Cobb's angle and preoperative local angle of the fracture vertebral body (P > 0.05). Compare to the control group, the patients who got CAMISS had more accuracy of pedicle screw, less blood loss, short immobilized time, lower postoperative fever, and better VAS score of the low back pain (t = 2.162 - 8.736, P < 0.05). The improvement of FSU Cobb's angle and local angle of the fracture vertebral body were better at control group after operation (13.8° ± 6.8° vs. 10.9° ± 5.5°, 11.0° ± 6.0° vs. 7.7° ± 4.8°, t = 2.108 and 2.610, P < 0.05). But there was no significant difference of the FSU Cobb's angle, or local angle of the fracture vertebral body between the two groups at post-operation and follow-up (P > 0.05).</p><p><b>CONCLUSIONS</b>CAMISS has the characteristics of fewer traumas, less bleeding, faster recovery, high accuracy of pedicle screws. It has comparable vertebral deformity correction and fixation result of the traditional open operation.</p>

<p><b>OBJECTIVE</b>To evaluate the effect of axial rotation of lumbar vertebrae on the accuracy of pedicle screw placement using the traditional method, as well as to assess the value of intraoperative three-dimensional (3D) navigation in improving the accuracy.</p><p><b>METHODS</b>Sixteen lumbar simulation models at different degrees of axial rotation (0°, 5°, 10° and 20°), with every four assigned with the same degree, were equally divided into two groups (traditional method group and intraoperative 3D navigation group). Random placement of pedicle screws was carried out, followed by CT scan postoperatively. Then the outer pedicle cortex contours were depicted from reconstructed sectional pedicle images using Photoshop. The accuracy of pedicle screw placement was evaluated by determining the interrelationship between screw trajectory and pedicle cortex (qualitative) and measuring the shortest distance from pedicle screw axis to outer cortex of the pedicle (quantitative).</p><p><b>RESULTS</b>Eighty pedicle screws were implanted respectively in each group. In traditional method group, statistical difference existed in the accuracy of pedicle screw placement at different axial rotational degrees (P < 0.05). With degrees increasing, the accuracy declined. The accuracy of intraoperative 3D navigation group was higher than traditional method group in vertebrae with axial rotation (P < 0.01). In qualitative evaluation, the accuracy of the two methods had statistical difference when the degree was 20°, and in quantitative evaluation, statistical difference existed in 5°, 10° and 20° of vertebral axial rotation.</p><p><b>CONCLUSIONS</b>Screw malposition can be caused by vertebral axial rotation in lumbar spine using traditional method. Accuracy of pedicle screw placement declines with the increase of axial rotational degrees. However, the accuracy can be improved by using intraoperative 3D navigation.</p>

<p><b>BACKGROUND</b>The complicated anatomy of the cervical spine and the variation among pedicles reduces the accuracy and increases the risk of neurovascular complications associated with screw implantation in this region. In this study, we compared the accuracy of cervical (C2-C7) pedicle screw fixation assisted by X-ray fluoroscopy, computed tomography (CT)-based navigation, or intraoperative three-dimensional (3D) C-arm navigation.</p><p><b>METHODS</b>This prospective cohort study was performed in 82 consecutive patients who underwent cervical pedicle screw fixation. The accuracy of screw insertion was assessed by postoperative CT scan with 3D reconstruction. The accuracy of screw insertion was assessed as: excellent (screw completely within pedicle); acceptable (≤ 1 mm screw outside pedicle cortex); poor (> 1 mm screw outside pedicle cortex).</p><p><b>RESULTS</b>A total of 145 screws were inserted in 24 patients who underwent C-arm fluoroscopy. Of these, 96 screws (66.2%) were excellent, 37 (25.5%) were acceptable, and 12 (8.3%) were poor. One hundred and fifty-nine screws were inserted in 29 patients in the CT-based navigation group. Among these, 141 (88.7%) were excellent, 14 (8.8%) were acceptable, and 4 (2.5%) were poor. A total of 140 screws were inserted in 29 patients in the intraoperative 3D C-arm navigation group, of which 127 (90.7%) were excellent, and 13 (9.3%) were acceptable. No severe or permanent neurovascular complications associated with screw insertion were observed in any patient.</p><p><b>CONCLUSIONS</b>CT-based and intraoperative 3D C-arm navigation were similarly accurate, and were both significantly more accurate than C-arm fluoroscopy for guiding cervical pedicle screw fixation. They were able to accurately guide the angle and depth of screw placement using visual 3D images. These two techniques are therefore preferable for high-risk cervical pedicle screw fixation. The ease and convenience of intraoperative 3D C-arm navigation suggests that it may replace virtual-fluoroscopy and CT-based navigation systems in future clinical applications.</p>

<p><b>OBJECTIVE</b>To evaluate the feasibility and accuracy of cervical (C(2)-C(7)) pedicle screw fixation assisted by C-arm fluoroscopy or 3D navigation system (CT-based navigation system or Iso-C 3D navigation system).</p><p><b>METHODS</b>One hundred and forty-five cervical pedicle screws inserted with C-arm fluoroscopy and 187 pedicle screws inserted with 3D navigation system were observed by postoperative CT or Iso-C 3D scan. The process of navigation was investigated.</p><p><b>RESULTS</b>In the 145 screws inserted with C-arm fluoroscopy, 133 screws are accurate (91.7%), and in the 187 screws inserted by 3D navigation system, 183 screws are accurate (97.9%) (chi2 = 6.705, P = 0.010), and the 4 misplaced screws occurred in the early stage of navigation system application. The process of navigation was investigated in twenty-five patients. With CT-based navigation, the mean time for registration and surface matching was 3.5 minutes; with Iso-C 3D navigation, the mean time for images collection and transfer was 6.2 minutes. The mean time for screw-marker insertion was 2 minutes.</p><p><b>CONCLUSIONS</b>3D navigation system can increase accuracy of cervical pedicle screw fixation obviously.</p>

RESUMO

Objective To explore the clinical characteristics and the remedial measurement of measles complicated with severe pneumonia and the acute respiratory distress syndrome(ARDS) in children.Methods From Jan 2004 to Jul 2007,clinical data of 32 cases with measles complicated with severe pneumonia and ARDS in PICU of Chongqing Children's Hospital were retrospective analyzed.In addition,the ratio of partial pressure of oxygen in arterial blood fractional concentration of oxygen in inspired gas[pa(O2)/FiO2] between the death group and survival group,the mortality rates between the hormone therapy group and control group were performed.Results Sixteen cases were cured and 16 cases were died,with the mortality rate 50%.The ratio of pa(O2) /FiO2 before mechanical ventilation in death group was lower than that in the survival group [(11.93?1.67) kPa,(21.77?1.16) kPa,respectively,P0.05).Conclusions ARDS is a severe complication in children with measles pneumonia and associated with a high mortality,which requires early diagnosis and intervention to improve survival rate and prognosis.The ratio of pa(O2)/FiO2 may provide reference to evaluation for severity and prognosis of ARDS.

RESUMO

Objective To investigate the effect of valsartan (angiotensinⅡtypeⅠreceptor antagonists) on neointimal proliferation and expression of CD34 after angioplasty in rabbits.Method Twenty-four male New Zealand White rabbits were randomly divided into three groups:the control group,fed up with common diet;the model group and the valsartan group,fed up with hypercholesterolemic diet for 4 weeks,f then and ballon angioplasty.At 4 weeks after operation,the model group was fed up with common diet,whereas the valsartan group was fed up with the admixture of valsartan 10 mg?kg~(-1)?d~(-1) and common diet.All the rabbits were killed at the end of the 12th weeks.The abdominal aorta was performed with pathologic and morphologic analysis,and expression of CD34 in endothelial cells was analyzed with immunohistochemical method.Results Compared with the model group,the neointimal thickness and area of the valsartan group decreased by 56.58%and 66.81%, respectively.The expression of CD34 of the valsartan group was significantly higher (P

<p><b>OBJECTIVE</b>To evaluate the feasibility and accuracy of cervical spine pedicle screw fixation assisted by X-ray fluoroscopy and CT-based navigation system.</p><p><b>METHODS</b>145 cervical pedicle screws were placed assisted by X-ray fluoroscopy and 159 cervical pedicle screws were placed assisted by CT-based navigation system. Screw positions were evaluated by postoperative CT scans or C-arm X-ray 3 dimensional reconstruction.</p><p><b>RESULTS</b>In the computer- assisted group, 155 (97.5%) screws were found completely within their pedicles compared with 133 (91.7%) correctly placed screws in the X-ray assisted group (P < 0.05). The process of navigation investigated in twenty patients showed that the mean time for registration and surface matching was 3.5 (range from 2 to 8 minutes) minutes and the mean time for screw-marker insertion was 2 (range from 1 to 3.5 minutes) minutes with the mean position deviation 0.31 mm (0.12-0.56 mm) per screw. Only twice C-arm fluoroscopy were performed to verify the accuracy of one screw position intraoperatively. No severe complications like neurovascular lesion occurred in both groups.</p><p><b>CONCLUSION</b>CT-based navigation system can increase the accuracy of cervical pedicle screw implantation significantly.</p>

<p><b>BACKGROUND</b>To explore the effect of human osteopontin (hOPN) on the proliferation, transmigration and expression of matrix metallproteinase-2 (MMP-2) and matrix metallproteinase-9 (MMP-9) in osteosarcoma (OS) cells in vitro.</p><p><b>METHODS</b>The prokaryotic-expression vector of hOPN was produced. hOPN was then subcloned into E. coli BL21 (DE3) cells and purified with ProBond trade mark Columns. The proliferation, cell cycle and the expression of cyclin A in OS cells were investigated by using MTT assay, flow cytometry and Western blot respectively. The transmigration of OS cells was checked by using transwell cell culture chamber. The micro-pore-filter-membrane system was used to study the chemiotaxis of hOPN to OS cells. The levels of total protein were examined according to Coomassie Brilliant Blue manuals. The expression of MMP-2 and MMP-9 were evaluated by detecting the volume of degradation of gelatin on SDS-PAGE gel.</p><p><b>RESULTS</b>The prokaryotic-expression vector of hOPN and purified hOPN protein were achieved hOPN promoted OS cells proliferation in a dose-dependent manner, and stimulated cyclin A expression in OS cells to accelerate cell division cycle. hOPN facilitated the trans-membrane migration of OS cells. hOPN also enhanced the secretion of MMP-2 and MMP-9 in OS cells.</p><p><b>CONCLUSION</b>hOPN could stimulate cyclin A expression in OS cells. hOPN has chemiotaxis to OS cells and increases their transmigration. hOPN enhances the secretion of MMP-2 and MMP-9 in OS cells.</p>
